Thank you for taking the time to complete our Liability Release Form. We ask that all riders (members, guests, event attendees) complete this form if they plan to attend any ESV Group Rides during the COVID-19 Pandemic.

FULL VERSION OF ESV LIABILITY RELEASE AGREEMENT

COVID-19 RIDE REQUIREMENTS
After conducting a survey of ESV members and ride participants, we are making the following guidelines mandatory for anyone attending an ESV Group Ride. If you are unable or unwilling to follow these guidelines, please do not attend an ESV Group Ride. These requirements are also in line with the current CDC guidelines for outdoor meetings and activities.
